Required enrollment documents

To ensure a smooth enrollment process, we ask parents to submit a few important documents at the time of registration. These help us get to know your child and create a safe, well-prepared environment for learning.

Please have copies of your child’s birth certificate, immunization records, emergency contact information, and any relevant medical forms. Our team is happy to guide you through each step and answer any questions along the way.

Our Policies and Procedures Review Form ensures that every family understands our center’s guidelines and daily practices. By reviewing and signing this form, parents acknowledge our commitment to maintaining a safe, respectful, and nurturing environment for all children.

Our Daycare Financial Agreement outlines tuition details, payment schedules, and enrollment policies. It helps ensure transparency and mutual understanding between our center and each family, supporting a smooth and consistent childcare experience.

The Emergency Medical Authorization form allows us to act quickly if your child ever needs urgent medical attention. By completing this form, you give our staff permission to seek immediate care, ensuring your child’s safety and well-being at all times.

The USDA Child Enrollment Form helps us provide nutritious meals and snacks for every child in our care. By completing this form, families ensure accurate meal planning and compliance with federal nutrition program guidelines.

The Development History form helps us understand your child’s unique growth, interests, and learning style. By sharing this information, you allow our teachers to better support your child’s development and create a personalized learning experience.
